Member is entitled to a comprehensive eye exam. |
Eyeglass Lenses

Single Vision, Bifocal, and

Generous Allowances for Lenticular and Progressive Lenses. |
Eyeglass Frames
(Once every 12 or 24 months) |
Members may choose any frame.
Frame allowances are applied toward the purchase of any frame in a provider’s collection.* |
Contact Lenses
(Once every 12 or 24 months) |
Members who elect contact
lenses in lieu of glasses may choose to apply the contact lens allowance toward the
purchase of any lenses. The contact lens allowance may also be applied toward any
professional fitting fee charged by a provider. |

Can Members purchase contact lenses and eyeglasses?

Members may purchase either contact lenses or
frames and eyeglass lenses in a benefit period.

How are contact lens fitting fees paid?
|
|
When a Member has an exam where he or she will be
purchasing contacts, there may also be a contact lens fitting fee required
by his or her doctor. Members have the option of using their contact lens
allowance toward the cost of the fitting fee, or they may elect to pay for
the fitting fee and use all of the allowance toward the purchase of their contacts.

Are there any brand restrictions for frames or
contact lenses?

No. Members may select any frame or contact
lens offered by the provider and AlwaysCare will pay up to the plan
allowance. Choose the frame that fits your style and personality!

Do Members have to purchase their
materials and have their eyes examined at the same location?
|
|
No. AlwaysVision offers Members the flexibility
to see any vision provider and purchase materials at any location they choose.
